New Horizon Center offers Home Base Facilitation Services for individuals who have been approved for the Home Base Program. The facilitator works with families and individuals to develop an Individual Service Plan that can provide a full spectrum of services and choices. The plan is developed and customized for the individual’s specific needs. Additionally, the facilitator coordinates resources to help the individual remain within the family structure.
