Home Based Self Directed Assistance

New Horizon Center offers Home Base Service Facilitation (HBS) for individuals who have been approved for the Home Base Program. The self-directed assistant works with families and individuals to assist with:

SSI benefits
Medicaid issues
Personal support workers (PSW) tasks
Funding allocation
Home modification and adaptive equipment
Access financial management
Service agreements
Employer of record
Review of a personal plan
Seeking residential placement
Acquiring technology equipment
