Home Based Self Directed Assistance

New Horizon Center offers Self-Direct Assistance Services as a Family-Based model only. We are here to
provide assistance to those receiving the Home-Based waiver in order to train them and assist them
with the following
 SSI benefits
 Medicaid issues
 Personal support workers (PSW) tasks
 Funding allocation
 Home modification and adaptive equipment
 Access financial management paperwork/updates
 Service agreements for personal support workers/ CDS programs
 Employer of record documents
 Acquiring technology equipment
 Research for programs in the community
We complete visits as requested per the family and use of restraints by our staff is prohibited.
Whenever visits are completed by any of our SDA’s our staff should always have family, PSW’s or other
case managers present (if our visits are completed at a CDS program).
